Account registration for new users
MPC Submission Form Instructions:
- Download and save the form on your computer (as in your downloads folder).
- Open in WORD.
- Check all boxes next to the services you need and fill in the required fields.
- Talk to a staff member before requesting special requests then list on your form.
- For paraffin or OCT blocks, fill in the table with cassette IDs or list in Excel.
- Email the completed form (and Excel when used) to molecular@ufl.edu.
